REQUIREMENTS

Students are eligible for enrollment in West Town Academy if they are Chicago residents, 17-21 years of age, have dropped out of Chicago Public Schools, have earned at least four high-school credits, and complete required paperwork. The following paperwork will be required from each student prior to enrollment. All forms or photocopies must be on file prior to beginning classes.

· Birth Certificate

· Proof of Address

· Photo ID

· Health/Immunization Records

· Drop Letter from previous high school

· Emergency Contact Form

· Photographic Release Form

· Lunch Application Form
